IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

Form 1095-C for the 2025 tax year is available upon request. This form provides information about the health insurance coverage we offered you during the year. While the employer mandate of the Affordable Care Act is still in effect, the individual mandate was repealed by Congress, so you do not need to wait to receive this form to file your taxes.
